Older Adults, Caregivers, and Discharge Planners: This web page allows you to find contact information for your local Care Coordination Unit (CCU), using your home address, for any of the following services: Home Delivered Meals; Caregiver Services (select Caregiver Resource Center); Adult Protective Services; Information and assistance, benefits, and anything else not listed (select Aging and Disability Resource Network).

Contacting the CCU will initiate the process to determine eligibility and connection to those services.

Managed Care Organizations (MCOs): You can use this web page to find meal providers for your clients, and make referrals to the proper provider for AgeOptions supported home delivered meal service.
