Older Adults, Caregivers, and Discharge Planners: Select a service from the dropdown menu below, then enter your home address to find contact information for the service provider in your area.

For Home Delivered Meals, select Community Care Program/In-home services/Home Delivered Meals.

For Caregiver Services, select Caregiver Resource Center.

To report cases of abuse, neglect, self-neglect and/or financial exploitation of older adults (60+) or adults with a disability, select Adult Protective Services.

For any desired service that is not listed, such as Information and Assistance or public benefits enrollment, select Aging and Disability Resource Network.

Managed Care Organizations (MCOs): You can use this web page to find meal providers for your clients, and make referrals to the proper provider for AgeOptions supported home delivered meal service.
